First time visitor to a CityROCK gym
Everyone who enters any CityROCK gym must fill out our liability waiver. This includes climbers, spectators and parents.
It is preferable that you fill out the waiver before coming to the gym so as to avoid delays and congestion.
Over 18?
Please fill out all the required fields. Bring your government-issued ID or Drivers Licence for verification.
Under 18?
For children under 18 years of age, the liability waiver must be signed by their parent or legal guardian*. If the parent or guardian will not be present, please bring a photocopy of the adult’s government-issued ID or Driver’s license for verification.
Note
A waiver will need to be signed for each location before first visit at that respective location.
*Please note: legal guardians must present proof of guardianship to sign a child’s waiver.
